Please download, fill out and print the following form with all your
information and mail it with your parts to:

Brooksville, FL 34604.
Please include any photos or other helpful information that you think
will be needed. Any jobs that come in without this form will be set
aside until we receive your information. When sending us a plate system
to be flowed we will need your plate lines and solenoids. If you are
sending us a manifold for flowing we need the system installed on the
intake without the carbs.
